Inside the body with Epitalon
Epitalon is a tiny lab-made chain of four amino acids (building blocks of proteins), copied from the make-up of an extract from cow pineal glands. It is not an approved medicine in the US, and we found no approval by any other regulator. Most of the research comes from one Russian group, in cells and animals, with a few small human studies. It has never been tested for safety in a proper trial.

Copied from a pineal gland extract
Epitalon was designed by looking at which amino acids are in epithalamin, an extract made from cow pineal glands. The pineal gland is a small gland deep in the brain that makes the sleep hormone melatonin. Epitalon is a four-piece copy made in a lab.

Epithalamin is a polypeptide complex extracted from bovine pineal gland; epitalon (Ala-Glu-Asp-Gly) is a tetrapeptide synthesised on the basis of the amino-acid content of that extract and is described as a synthetic analogue with similar biological effects. FDA considers the two different substances. A 2025 review reports that in 2017 the tetrapeptide was detected by LC-MS in a pineal gland polypeptide complex solution. The idea that it acts as a pineal regulator rests largely on work from the originating group.[src][src]

What happens after it enters the blood is unknown
Nobody has measured how fast the body breaks epitalon down or where it goes. Very short peptides like this are often broken up quickly, but that has not been shown for epitalon.

FDA found no pharmacokinetic data for epitalon free base or acetate in humans, and no acute-toxicity or repeat-dose toxicity studies. The only distribution data are a single report in pregnant rabbits given one dose of fluorescent-labelled epitalon, in which fluorescence was seen one hour later in maternal and fetal liver, lungs, stomach and intestines, but the study did not show the signal came specifically from labelled epitalon. A 2025 review states that information on the peptide's safety is missing from the published work and notes, as a general point, that short peptides such as epitalon are typically unstable and degrade rapidly in vivo.[src][src]

Melatonin and stress hormone in old monkeys
In old female monkeys, ten days of epitalon raised evening melatonin and made their daily cortisol (stress hormone) rhythm look more normal. In young monkeys, melatonin did not change, but cortisol went down in both morning and evening. A separate test on rat pineal glands in a dish found no direct effect on melatonin.

In female rhesus monkeys given epitalon by intramuscular injection for 10 days, evening serum melatonin rose more than 4-fold and evening cortisol fell by about 30% in the old animals; in young animals melatonin did not change significantly, while cortisol fell in both morning and evening samples (Goncharova 2001, as summarised by FDA). FDA noted that the functional relevance for sleep was unclear and that no sleep-behaviour or EEG studies were found. In rat pineal glands perifused in vitro, epitalon did not change basal or isoproterenol-stimulated melatonin release in young or old animals (Djeridane 2003), so a direct action on the pineal is not supported and the mechanism in vivo is unknown.[src][src][src]

A small human study on a melatonin marker
One small study gave women who work night shifts either epitalon or a dummy spray under the tongue. In women who started with low levels of a melatonin by-product in urine, the level went up in the epitalon group. The study did not show that anyone slept better.

Ivko 2021 was described by FDA as a randomised placebo-controlled study in healthy women aged 40-50 (n = 75) mainly working night shifts. Of 40 women with low urinary 6-sulfatoxymelatonin, those given sublingual AEDG for 20 days had a 1.7-fold rise, which the authors described as restoring age-appropriate values, while placebo showed no change. The authors also reported changes in clock-gene expression in blood cells and better psycho-emotional condition in 83% versus 25% on placebo, but did not say how this was measured. FDA noted blinding was not specified, duration was short, the sample was small, and no sleep outcomes were assessed. It has not been shown to treat insomnia.[src][src]

Telomerase switched on in cultured cells
Telomeres are protective caps at the ends of our DNA that get shorter as cells divide. A 2003 lab report said epitalon made human cells in a dish switch on the enzyme that rebuilds these caps. This was done in cell cultures only, and it has not been shown to happen in a living person.

Khavinson et al. (2003) reported that adding epitalon to telomerase-negative human fetal fibroblast cultures induced expression of the catalytic subunit (hTERT), telomerase activity and telomere elongation. FDA describes these as cell-culture findings, and the reports it cites come from the originating group. No study has shown telomere lengthening in living humans. FDA also notes the other side of this mechanism: longer telomeres are generally associated with higher cancer risk, so continuous telomerase activation is a theoretical carcinogenicity concern.[src][src]

Mouse studies of lifespan and tumours (animal only)
In female mice given epitalon on a repeating schedule for life, average lifespan did not change, but the longest-lived mice lived a little longer and fewer developed one kind of blood cancer. Other mouse studies reported fewer breast tumours. These are mouse results only.

In female Swiss-derived SHR mice (54 per group) treated subcutaneously on 5 consecutive days each month from 3 months of age until natural death, epitalon did not change mean lifespan, food intake or body weight, but increased maximum lifespan by 12.3% and lifespan of the last 10% of survivors by 13.3%, reduced chromosome aberrations in bone marrow by 17.1%, left total spontaneous tumour incidence unchanged and reduced leukaemia 6.0-fold (Anisimov 2003). In HER-2/neu transgenic mice, epitalon reduced the number and size of mammary tumours and lowered HER-2/neu mRNA 3.7-fold in tumours (Anisimov 2002). FDA judged these studies too limited for safety conclusions: fixed regimen with no dose-response, females only, exposure of at most about 5.5 months, and chromosome-aberration comparisons that relied on an unvalidated historical control group of four mice.[src][src][src]
